Payment

Payment for professional services is due at the time treatment is provided unless an automatic payment plan is arranged. Every effort will be made to provide a treatment plan which fits your timetable and budget and gives you the best possible care. We accept cash, checks, american express, visa, master and discover card.

Insurance

If we receive all of your current insurance information on the day of your appointment we will be happy to file your claim for you. Please understand that we file dental claims as a courtesy to our patients. We do not have a contract with your insurance company, only you do. You must be familiar with your insurance benefits. We are not responsible for how your insurance company handles its claims or for what benefits they pay. We can only assist you in estimating your portion of the cost of treatment. We at no time guarantee what your insurance will or will not do with each claim.

No insurance pays 100% of all procedures. Dental insurance is meant to be an aid in receiving dental care. The percentage paid is usually determined by the type of contract you or your employer has set up with the insurance company. Benefits are not determined by our office. You may have noticed that sometimes your dental insurance will reimburse you or the dentist at a lower rate than the dentist’s actual fee. Frequently insurance companies state that the reimbursement was reduced because your dentist’s fee has exceeded the usual, customary, or reasonable fee (“UCR”). A statement such as this gives the impression that any fee greater than the amount paid by the insurance company is unreasonable or above what most dentists in the area charge for a certain service. This can be misleading and simply is not accurate. Insurance companies set their own fee schedules and each company uses a different set of fees they consider allowable.

Your Personal Information

Please keep us informed of any changes such as address, phone numbers, insurance policy or company, or change of employment. Upon your arrival we request that you please turn off both your cell phone and pager. As a courtesy to our patients and staff and as a safety precaution, we respectfully request that children without an appointment do not accompany you to your scheduled appointment. If you find that you have a conflict, we will be happy to reschedule you for a better time.
